A spouse or child on the Digital Nomad visa
A spouse or a child can come with you on their own designated-activities status, for the same six months. It is not a share of your status. What it allows is daily life as your dependent, not the remote work you are here to do.
Who can come
The status is for a spouse or child accompanying a person who will stay in Japan for not more than six months and do international remote work. The permitted activity is daily life as the supported spouse or child of that person. The stay is six months, and it is not extended. That activity is the one on the Immigration Services Agency page and the Ministry of Foreign Affairs page.
Their nationality is notification 54: a visa-exempt country or region, 73 names on the chart. The tax treaty is not part of their test, so they can be from a place on that longer list even when you, the worker, have to be on the list of 51. The published requirements for a spouse or child do not include the ¥10 million income. That figure stays with the worker.

- 51 for you, 73 for them.
The insurance
Each of them needs private medical insurance covering death, injury and illness during the stay, with treatment cover of at least ¥10 million, for the period of the stay. Family cover on your own policy is accepted when you submit papers that show what that cover includes. Credit-card cover is accepted on the same terms, when the papers show it.
The papers
At the embassy or consulate, a spouse or child files a visa application form with a photo and their passport. A certificate of eligibility is optional. The ministry says that presenting one lets you omit the activity plan and the insurance papers. You still file papers proving the relationship, a marriage certificate is the example, and a copy of the passport of the spouse or parent who holds the Digital Nomad visa.
Without the certificate, add the activity plan for their stay, and the insurance papers: the certificate and the policy summary, or the credit card and the cover that comes with it. When the cover is your family policy, add the papers that show its scope.
The certificate itself, applied for in Japan before a new entry, asks for the application form, a photo, a reply envelope, the activity plan unless the form already has it, a paper proving the relationship, the insurance explanation and the policy, and a copy of the passport of the spouse or parent who will hold the digital-nomad status. There is no income certificate on the family's list.

A child born in Japan
A child born in Japan who will be the dependent of the digital-nomad parent applies for permission to acquire a status of residence, not for a certificate of eligibility. The papers are the application form, a document proving the birth, the child's passport presented, the activity plan unless the form has the detail, a paper proving the relationship such as a birth certificate, and the same insurance papers, including family cover on the parent's policy when the scope is shown. The regional immigration office can ask for other papers, or waive some of the ones the ordinance lists. The information centre is 0570-013904, or 03-5796-7112 from overseas.
